The training’s target groups are experts in addictionology, applied health psychology, general practitioners (general practitioners), internal medicine, infant and pediatrics, health services, geriatrics, gerontology, child psychology, and child and adolescent psychiatry.

The training aims to help healthcare providers navigate the tasks they share with the social care system.

It will focus on the interface between the two care systems regarding roles, competencies, and responsibilities.

How can cross-sectoral cooperation result in more efficient patient pathways?

How can better results be achieved in prevention, compliance, adherence, health maintenance and recovery, crisis and crisis management?

We will present the results of intersectoral collaborations, practices, and opportunities to support each other’s services and effective care for patients.

Thematic areas:

  • Content and characteristics of social services, identification of typical clientele
  • Functioning of the vulnerability detection and signaling system, legal framework
  • From signaling to practical action – through presenting a specific case, identifying roles and responsibilities, and scope of action
  • Health promotion in primary social services, effective chronic and acute conditions management
  • Critical points in the process and good practices in healthcare-deprived areas
  • Characteristics of care and support for adults and children, the role of rehabilitation and specialized care centers at the interface between the two care systems
  • Case studies:
  • Collaboration in the provision of health care in a nursing home, complemented by a specialized care center
  • Crisis management processes in family and child welfare services, family transition homes, and crisis centers linked to health services
  • Integrated operational models, complex services, interdisciplinary teams
